Why do I need a Photo ID Card?
All CMSA players from U11 to U18 and all CMSA Team Officials for U9 to U18 teams require a valid CMSA Photo ID card. This card must be present at the start of each game so that the referee can identify the players/coaches on the field.
What do I need to bring to my appointment?
- Bring yourself – you must be present during your appointment as we will be taking your picture for the card. The card will be printed right away.
- Bring identification/proof of age – we recommend players bring proof of age so that we can ensure we have your correct date of birth in our system.
- There is a $10.00 cost for new/replacement cards – If you have an old card that has expired, bring it to your appointment and there will be no charge for a new one.
